It's all about Ask and Bid. The quotes they can and are just like on the real. But inside the bar they move quite differently than in the tester.
In the tester Ask and Bid move in sync.
And they move asynchronously on the online chart.
Sometimes, even Asc and Bid go towards each other; it is not present in the Strategy Tester. More often, one stands and the other goes back and forth rapidly... - There is no such thing in the tester.
It's all about Ask and Bid. They can quote like on the real. But inside the bar they move very differently than in the tester.

If you have a floating spread, switch to real ticks mode in the MT5, switch to visualisation mode and look at how the spread changes.
