Draw levels above and below

1. I want a custom indicator which draws little horizontal levels above
and below price.

2. The indicator must repaint (redraw the lines) every tick.

3. There are 4 different lines.  One line should be 10 pips below market price and should be blue in color.
One line should be 10 pips above market price and should also be blue in color.  The other two lines
are 5 pips above and below, and should be green.  Here is a mock up http://i.imgur.com/W0PX3Qk.png

4. The lines should be short (about 5 candles back) , rather than going across the entire screen.

That's it.  Simple , right?
