User Interface (template) for an existing ea

Hello, I have an ea (in mt4 and mt5) that should be put in only one chart but take orders in different symbol. That's why I want to exploit the chart I am putting the ea on it as a simple graphical interface. It could be a template that will include my ea. I need only a simple user interface (please check the jpg below)
