Ok/Cancel buttons disappear when applying EA to chart

I recently came across a strange behavior: when I apply an EA to a chart, I don't see the Ok and Cancel buttons. This of course means I can't apply the EA to the chart, and the only thing I can do is click the X button to close the EA inputs window without applying. This is how it looks like: The

EA's opening double orders

I have a very strange problem and I'd really apreciate any help! I'm running several MT4 platforms on a WIndows 2003 server. Each platform has its own demo account with a different Expert Advisor. Each time an Expert needs to open a trade, it opens 2 identical trades. Please see attached screen. It
