Quick PHP File Fix

I had some code done here on oDesk and the developer ended up messing the code up intentionally. I need to have this code fixed for the website to work. The php file did work at one time and know that the dev went in there and messed everything up I need someone with enough experience to put everything back in place. The script is only about 20-30 lines of code.
