MT4 panel with code

I need a simple panel in mql4 code to put together with some strategies I created. As per the attached photo. The panel will have the buy and sell buttons and the option to increase or decrease the lot of that buy or sell. There is also a field below with the magic number that will be buy or sell. It will also show the name of 4 strategies and the side will show if they are TRUE or FALSE, to know if they are active or not.
