I got one a long time ago, about five years ago. I went to a place in St. Petersburg where they exchanged WMs and issued certificates. I sent them scans of my passport beforehand, as I remember. And at the point checked that I was me. I do not remember the $10 registration fee.
 
Checked: withdrawal to card - inactive, withdrawal by transfer - no problem. The passport is 'formal'.
