Can anyone explain why a Trade execution would just appear in MT4, has
happened to me three times now and I did not initiate the trade.

If it's a real account then contact your broker right-away. For demo-account try checking if any Expert Advisors or Scripts are active. Checking the Logs and Journals tabs may help.
