Text Box and Chart Lines

Require an indicator or script, that will read in CSV values from file and update chart frequently (on tick or at set intervals, whichever as long as it is relatively frequent). The first 5 CSV values in the CSV file will be instrument prices that should mark out horizontal lines on a chart; the lines should start at the current candle and extend to the right. These lines should also have their corresponding price level marked clearly, and it's required that they be visibly named.

 I also need a box with 5 values (top left of chart) that will also be updated by the same CSV file.

Hence for each instrument there will be a corresponding CSV file in the files folder (files created by another application), named identically to the instrument we are interested in, USDJPY.csv in this case. They will have the following format:

p.s. i do not know any C, so make it easy to edit the indicator, no fancy tricks, just straight forward code.
